Can't get info from form submission

Question

I created form and want into to send mail to my mail when a user submit a request. I found a php script which suites my form and its working fine with other form, but when I try to use that script for my form its not working as expected. This is my form in html-

And "is not working" means _what_? _Be precise_! What happens if you submit the form? Nothing? Do you get an error? Something strange happens? Does the universe implode? — arkascha, Jun 11 '16 at 18:37
Because you are not submitting form. You are using `` tag which just openes a contact.php page. You need to have either `` or `` element which will submit the form. — TheDrot, Jun 11 '16 at 18:39
Plus, `$from = "Customer"; $headers = "From:" . $from . "\r\n";` that is failing you. `mail()` is expecting an email address here. RTM http://php.net/manual/en/function.mail.php — Funk Forty Niner, Jun 11 '16 at 18:44

make sure the php script is in the same directory as the form
