From Middle High German swanz (“tail”), derived from the verb swanken (“to swing, to wobble”). The sense “penis” is already attested in late Middle High German. Compare German Schwanz (“tail; penis”), similar semantic shift as in e.g. Middle Armenian ձետ (jet).
